Individual and couple psychotherapy services are provided out of two convenient offices, located in Carefree and in Scottsdale (see office locations below).
I am in my Scottsdale office every week on Monday morning, and in my Carefree office Tuesday through Saturday.
Evening and Saturday hours are available in my Carefree office only. Both offices have good parking and private waiting areas.
I am not a provider for any Health Insurance, with the exception of MMSI. Please contact your Insurance carrier if you need to know their reimbursement policy for out of network services.

Please download and complete these forms and bring them with you to your first session with me. Everybody needs to read and sign the Policies and General Agreement for Services Form. If you are seeking couple therapy, both you and your partner need to fill out both the Couple's Biological Information Form and the Informed Consent For Patients In Couple's Therapy forms. If you are seeking individual psychotherapy, please fill out the Individual Biographical Information Form. Thank you.
